Erythromycin markedly raises domperidone exposure and both drugs prolong the QT interval, increasing the risk of torsades de pointes and sudden cardiac death. Domperidone labeling in Europe contraindicates co-administration with QT-prolonging drugs and potent CYP3A4 inhibitors. Risk is greater in older patients, those with cardiac disease, electrolyte disturbances or higher domperidone doses.

Domperidone and Erythromycin are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Miért jön létre
Erythromycin inhibits CYP3A4, the main metabolic pathway of domperidone, and both drugs block the hERG potassium channel, giving additive QT prolongation.
Teendő
Do not combine; choose a non-QT-prolonging antiemetic or prokinetic, or a different antibiotic such as one without CYP3A4 inhibition or QT effects. If erythromycin is essential, withhold domperidone for the course.
