Duvelisib inhibits CYP3A4 and can raise exposure to sensitive CYP3A4 substrates. Ergotamine is cleared mainly by CYP3A4, and higher levels carry a risk of ergotism with severe peripheral and cerebral vasospasm and ischemia.

Duvelisib and Ergotamine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
