Summary
Duvelisib inhibits CYP3A4 and can raise exposure to sensitive CYP3A4 substrates. Ergotamine is cleared mainly by CYP3A4, and higher levels carry a risk of ergotism with severe peripheral and cerebral vasospasm and ischemia.
What this interaction means in plain words
Duvelisib and Ergotamine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Duvelisib belongs to Oncology; Ergotamine belongs to Central Nervous System.
Why it happens
Duvelisib inhibits CYP3A4, reducing the first-pass and systemic metabolism of ergotamine.
What to do
Avoid combining ergotamine with duvelisib and choose a non-ergot acute migraine treatment. If it cannot be avoided, monitor closely for ischemic symptoms such as cold or numb extremities and chest pain.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
