Összefoglaló

Glucarpidase is an enzyme that cleaves the glutamate residue from folates and classical antifolates. Pemetrexed has a glutamate moiety and may therefore be hydrolyzed, which could lower its exposure and reduce antitumor effect if given close in time. The clinical relevance is uncertain because glucarpidase is used for methotrexate toxicity.

Mit jelent ez a kölcsönhatás közérthetően

Glucarpidase and Pemetrexed have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Glucarpidase belongs to Miscellaneous; Pemetrexed belongs to Oncology.

Bizonyítékszint: Low

Miért jön létre

Glucarpidase hydrolyzes the terminal glutamate of antifolates, potentially inactivating pemetrexed.

Teendő

Avoid giving pemetrexed within the period around a glucarpidase dose unless clinically required, and review the intended pemetrexed schedule if glucarpidase is used for methotrexate rescue.
