Tadalafil potentiates the hypotensive effect of nitrates and the combination can cause severe, potentially fatal hypotension, syncope or myocardial ischaemia. Because of tadalafil's long half-life, the risk persists for a prolonged period after a dose. Use is contraindicated in labeling.

Nitroglycerin and Tadalafil are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

PDE5 inhibition prevents cGMP breakdown while nitroglycerin raises cGMP via nitric oxide, producing exaggerated vasodilation.

Do not use nitrates in patients who have taken tadalafil in the previous 48 hours. If a nitrate is needed urgently after that interval, use it only under close medical supervision.
