Sintesi
Combining metoclopramide with antipsychotics increases the risk of extrapyramidal symptoms, including acute dystonia, parkinsonism, akathisia and tardive dyskinesia, and may raise neuroleptic malignant syndrome risk. Metoclopramide labeling warns against use with other dopamine antagonists. Older adults and long-term users are most at risk.
Cosa significa questa interazione in parole semplici
Antipsychotics (e.g., haloperidol) and Metoclopramide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Perché avviene
Additive central dopamine D2 receptor antagonism in the nigrostriatal pathway.
Cosa fare
Avoid the combination; use a non-dopamine-blocking antiemetic such as ondansetron (checking QT risk) or a prokinetic alternative. If unavoidable, restrict metoclopramide to short courses (under 5 days) and monitor for movement disorders.
Cosa chiedere al farmacista
