Sintesi
Apalutamide is a strong CYP3A4 inducer and can substantially lower pralsetinib exposure. This may lead to reduced antitumor efficacy of pralsetinib.
Cosa significa questa interazione in parole semplici
Apalutamide and Pralsetinib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Apalutamide belongs to Endocrine; Pralsetinib belongs to Oncology.
Perché avviene
Induction of CYP3A4 (and P-gp) by apalutamide increases the metabolic clearance of pralsetinib.
Cosa fare
Avoid the combination where possible and consider an alternative to apalutamide. If it cannot be avoided, labeling advises doubling the pralsetinib dose from day 7 of the inducer and returning to the original dose after the inducer has been stopped for at least 14 days.
