Sintesi
Carbamazepine is a strong inducer of CYP3A4, CYP2B6, CYP1A2 and P-glycoprotein and accelerates the clearance of irinotecan. The result is altered irinotecan and SN-38 disposition, with reduced exposure and possible loss of efficacy.
Cosa significa questa interazione in parole semplici
Carbamazepine and Irinotecan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Carbamazepine belongs to Central Nervous System; Irinotecan belongs to Oncology.
Perché avviene
Enzyme induction by carbamazepine increases metabolic clearance of irinotecan
Cosa fare
Avoid where possible by choosing a non-inducing alternative; if unavoidable, monitor clinical response (and drug levels where available) and consider a higher irinotecan dose only per specialist advice.
