Sintesi
Dichlorphenamide, a carbonic anhydrase inhibitor, causes hyperchloremic metabolic acidosis. Metformin carries a risk of lactic acidosis, and SGLT2 inhibitors carry a risk of ketoacidosis, so the combined acid-base burden is a concern.
Cosa significa questa interazione in parole semplici
Dichlorphenamide and Empagliflozin-metformin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dichlorphenamide belongs to Miscellaneous; Empagliflozin-metformin belongs to Endocrine.
Perché avviene
Additive acidosis through bicarbonate loss from carbonic anhydrase inhibition and the acid-base effects of metformin and empagliflozin.
Cosa fare
Monitor serum bicarbonate and electrolytes at baseline and periodically, and reassess therapy if metabolic acidosis develops or persists.
