Sintesi
Glyburide is metabolized by CYP2C9 and CYP3A4, both inhibited by miconazole. Increased levels can cause severe and prolonged hypoglycemia, and cases have been reported with miconazole. The risk is greater in elderly patients and those with renal impairment.
Cosa significa questa interazione in parole semplici
Glyburide and Miconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Glyburide belongs to Endocrine; Miconazole belongs to Anti Fungals.
Perché avviene
Miconazole inhibits CYP2C9 and CYP3A4, reducing glyburide clearance.
Cosa fare
Monitor blood glucose frequently and counsel on hypoglycemia symptoms. Reduce the glyburide dose or switch to a non-azole antifungal or alternative agent if feasible.
