作用のしくみ
Trop-2-directed antibody-drug conjugate. The humanized antibody binds Trop-2-expressing cancer cells and is internalized, releasing SN-38, a topoisomerase I inhibitor. SN-38 prevents re-ligation of single strand breaks, causing DNA damage, apoptosis and cell death.
成人の通常用量（添付文書の要約）
Triple-negative breast cancer or HR-positive/HER2-negative breast cancer: 10 mg/kg IV infusion on Days 1 and 8 of each 21-day cycle, as a single agent or with pembrolizumab; continue until disease progression or unacceptable toxicity; do not exceed 10 mg/kg

- Unsafe Can cause birth defects or embryo-fetal death: it contains SN-38, a genotoxic drug that targets dividing cells. No human or animal reproduction data. Check pregnancy status; use effective contraception during treatment and for 6 months after (men: 3 months).

- Unsafe No human milk data. The antibody part is large and poorly absorbed, but the SN-38 payload may enter milk. US label: do not breastfeed during treatment and for 1 month after the last dose.

- Can cause severe, life-threatening or fatal neutropenia. Withhold for ANC below 1500/mm3 or neutropenic fever; monitor blood counts; give primary G-CSF prophylaxis to patients at increased risk of febrile neutropenia; treat febrile neutropenia with anti-infectives without delay. Can cause severe diarrhea: monitor, give fluids and electrolytes, evaluate for infection and start loperamide; if severe, withhold until ≤ Grade 1 and reduce subsequent doses.

- Severe hypersensitivity reaction to TRODELVY.

- Neutropenia occurred in 64% of patients (Grade 3-4 in 48%); monitor ANC and give G-CSF prophylaxis to patients at increased risk.
- Severe diarrhea; evaluate for infection and start loperamide; fluid and electrolyte replacement.
- Serious hypersensitivity and infusion-related reactions including anaphylaxis; premedicate and observe for at least 30 minutes after infusion.
