概要
Phenytoin, formed from fosphenytoin, is a strong CYP3A4 inducer and can substantially lower ranolazine levels, causing loss of antianginal effect. Ranolazine labeling contraindicates CYP3A inducers.
この相互作用をわかりやすく説明すると
Fosphenytoin and Ranolazine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Fosphenytoin belongs to Central Nervous System; Ranolazine belongs to Cardiovascular.
なぜ起こるのか
Induction of CYP3A4 (and P-gp) increases ranolazine clearance.
対処法
Avoid the combination and choose another antianginal. Use a different anticonvulsant if the patient needs ranolazine.
