Para que é usado
Como funciona
Recombinant bacterial enzyme that hydrolyzes the carboxyl-terminal glutamate residue from folic acid and classical antifolates such as methotrexate, converting methotrexate to inactive metabolites DAMPA and glutamate.
Dose habitual em adultos (resumo da bula)
Toxic plasma methotrexate concentration with delayed clearance due to impaired renal function: 50 Units/kg as a single intravenous injection over 5 minutes; flush IV line before and after.
Gravidez e amamentação
- Gravidez
- Unknown No human or animal data. Glucarpidase is given to clear toxic methotrexate levels; methotrexate itself can cause fetal harm, so follow the methotrexate label. Give when clearly needed for methotrexate toxicity.
- Amamentação
- Unknown No data on milk levels or infant effects. It is given with methotrexate, which is not compatible with breastfeeding; follow the methotrexate advice.
Efeitos adversos relatados
- Paresthesia
- Flushing
- Nausea and/or vomiting
- Hypotension
- Headache
Quem deve ter cuidado
- Serious hypersensitivity reactions occurred in less than 1% of patients.
- DAMPA metabolite interferes with immunoassays, overestimating methotrexate concentration for up to 48 hours after a dose; a chromatographic method is preferred.
Informe ao farmacêutico ou ao médico sobre gravidez ou amamentação, doença renal ou hepática, alergias e todos os outros medicamentos ou suplementos que você usa.
