Rezumat
Febuxostat inhibits xanthine oxidase and can raise levels of azathioprine's active metabolite, potentially causing severe bone marrow suppression. Labeling advises against use together because no safe dosing strategy is established. Patients with renal impairment or low TPMT activity are at particular risk.

Azathioprine and Febuxostat are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Febuxostat inhibits xanthine oxidase, which is needed to inactivate 6-mercaptopurine, leading to accumulation of active thiopurine metabolites.
