摘要
Iodinated contrast can cause acute kidney injury, which reduces metformin elimination and leads to accumulation. This increases the risk of metformin-associated lactic acidosis, especially in patients with reduced renal function.

Hold metformin at the time of or before the procedure in patients with eGFR 30-60, liver disease, alcoholism, heart failure, or intra-arterial contrast. Re-check renal function 48 hours afterward and restart only if it is stable.
