摘要
Both tiotropium and umeclidinium are long-acting inhaled antimuscarinics, so combining them duplicates therapy. This increases anticholinergic effects such as dry mouth, urinary retention, constipation, tachycardia and worsening narrow-angle glaucoma without added benefit.
用白話說明這項交互作用的意義
Tiotropium and Umeclidinium bromide / vilanterol inhaled can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
為什麼會發生
Duplicate long-acting muscarinic antagonism producing additive anticholinergic effects.
處置方式
Avoid concurrent use and keep only one long-acting antimuscarinic. Stop one product and monitor for anticholinergic effects if the combination was used.
