Summary

Hydroxyurea is myelosuppressive and can impair immune responses. A live oral adenovirus vaccine may therefore be less effective and could cause vaccine-related infection.

What this interaction means in plain words

Adenovirus types 4 and 7 live and Hydroxycarbamide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Immunosuppression from hydroxyurea impairs the vaccine response and permits replication of live virus.

What to do

Avoid live vaccines during hydroxyurea therapy. Defer vaccination until immune recovery or use a non-live alternative where available.
