Summary

Pentobarbital induces CYP3A4 and may lower alprazolam concentrations, reducing its anxiolytic effect over time. At the same time both drugs are CNS depressants, so combined use increases sedation and the risk of respiratory depression.

What this interaction means in plain words

Alprazolam and Pentobarbital can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Barbiturate induction of CYP3A4 lowers alprazolam exposure, while both agents produce additive GABAergic CNS depression.

What to do

Avoid combining where possible; if used, monitor sedation, respiratory status and clinical response to alprazolam. Adjust the alprazolam dose if effect is lost or when the barbiturate is stopped.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
