Summary

Barbiturates add to benzodiazepine CNS and respiratory depression and also induce CYP3A4, which can lower alprazolam levels. The net result is unpredictable, with risks of oversedation and of reduced alprazolam effect, and abrupt changes can cause withdrawal.

What this interaction means in plain words

Alprazolam and Secobarbital can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Alprazolam belongs to Central Nervous System; Secobarbital belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive GABA-A mediated CNS depression plus CYP3A4 induction by the barbiturate increasing alprazolam clearance.

What to do

Avoid combining where possible; if both are required, use the lowest doses and monitor sedation and respiration. Watch for loss of alprazolam effect and adjust the dose accordingly.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
