Summary
Amiloride and thiazide diuretics are commonly combined deliberately, because amiloride offsets thiazide-induced potassium and magnesium loss and adds diuretic effect. Hyperkalemia can still occur, especially in renal impairment, and hyponatremia or volume depletion may develop.
What this interaction means in plain words
Amiloride and Chlorothiazide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Amiloride belongs to Cardiovascular; Chlorothiazide belongs to Cardiovascular.
Why it happens
Amiloride blocks ENaC in the collecting duct and conserves potassium, while chlorothiazide inhibits the sodium-chloride cotransporter and promotes potassium loss, so the effects are complementary.
What to do
Monitor serum potassium, sodium and renal function periodically, particularly after dose changes. Reassess if renal function declines or other potassium-raising drugs are added.
