Summary
Losartan and amiloride both raise serum potassium, and the potassium-lowering effect of hydrochlorothiazide only partly offsets this. Hyperkalemia can occur, especially in patients with renal impairment, diabetes or older age.
What this interaction means in plain words
Amiloride and Hydrochlorothiazide-losartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amiloride belongs to Cardiovascular; Hydrochlorothiazide-losartan belongs to Cardiovascular.
Why it happens
Angiotensin receptor blockade reduces aldosterone-mediated potassium excretion while amiloride blocks the epithelial sodium channel, additively causing potassium retention.
What to do
Check serum potassium and creatinine within the first week or so and periodically thereafter. Avoid potassium supplements and salt substitutes, and stop amiloride if potassium becomes elevated.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
