Summary
Indomethacin can blunt renal potassium excretion while amiloride is potassium-sparing, so combined use can cause hyperkalemia. NSAIDs may also reduce the diuretic and antihypertensive effect of amiloride and worsen renal function.
What this interaction means in plain words
Amiloride and Indomethacin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amiloride belongs to Cardiovascular; Indomethacin belongs to Musculoskeletal.
Why it happens
Additive potassium retention: amiloride blocks ENaC and indomethacin suppresses prostaglandin-mediated renin and aldosterone and renal potassium excretion.
What to do
Avoid the combination where possible; if used, monitor serum potassium and creatinine within the first week and periodically after, especially in the elderly, diabetics or those with renal impairment.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
