Summary
Ketoprofen can blunt the effects of amiloride and increases the risk of hyperkalemia, which is especially significant in renal impairment, diabetes or older age. Acute kidney injury can also occur.
What this interaction means in plain words
Amiloride and Ketoprofen can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amiloride belongs to Cardiovascular; Ketoprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of prostaglandins reduces renin and aldosterone and lowers renal perfusion, which adds to amiloride's potassium-sparing effect.
What to do
Avoid the combination in patients with renal impairment; otherwise monitor serum potassium and creatinine within the first week and periodically thereafter.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
