Summary
Amiodarone inhibits CYP2C9 and can raise levels of hydantoin anticonvulsants, as is well documented with phenytoin. Ethotoin may behave similarly, causing nystagmus, ataxia and sedation. Phenytoin-type drugs may in turn reduce amiodarone levels.
What this interaction means in plain words
Amiodarone and Ethotoin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Amiodarone inhibits CYP2C9 and related enzymes, reducing hydantoin metabolism; the effect is extrapolated from phenytoin.
What to do
Monitor for signs of anticonvulsant toxicity and seizure control, and reduce the ethotoin dose if toxicity appears. Monitor amiodarone effect as well.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
