Summary
Both glecaprevir and pibrentasvir inhibit P-glycoprotein, and amiodarone is a P-gp substrate and inhibitor, so exposure of either may rise. Increased amiodarone levels may cause bradycardia and QT prolongation. Symptomatic bradycardia has been reported with amiodarone and direct-acting antivirals, mostly with sofosbuvir regimens.
What this interaction means in plain words
Amiodarone and Glecaprevir / pibrentasvir can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Mutual P-glycoprotein inhibition altering efflux of both drugs.
What to do
Monitor heart rate and ECG during the first weeks of co-therapy. Reduce amiodarone dose if bradycardia or QT prolongation develops.
