Summary

Miconazole is a CYP3A4 inhibitor and, although vaginal absorption is low, it can raise levels of CYP3A4 substrates. Amiodarone is a CYP3A4 substrate with a narrow therapeutic margin, so increased exposure could add to QT prolongation and bradycardia. Clinically significant effects are uncommon with short vaginal courses.

What this interaction means in plain words

Amiodarone and Miconazole vaginal have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Inhibition of CYP3A4-mediated amiodarone metabolism by systemically absorbed miconazole.

What to do

Short vaginal courses generally need no change. In patients with long-term use or other QT risk factors, monitor heart rate and ECG for QT prolongation.
