Summary

Calcium salts given parenterally can raise ionized calcium and partly oppose the blood pressure lowering effect of amlodipine. Intravenous calcium has been used to reverse calcium channel blocker toxicity, which illustrates the antagonism. Oral calcium supplements rarely cause a meaningful loss of antihypertensive effect.

What this interaction means in plain words

Amlodipine and Calcium chloride can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Increased extracellular calcium pharmacodynamically opposes calcium channel blockade at vascular smooth muscle.

What to do

Monitor blood pressure when intravenous calcium is given to patients taking amlodipine, and expect a reduced antihypertensive response. Avoid giving unnecessary calcium boluses and check serum calcium if hypercalcemia is suspected.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
