Summary
Asenapine can cause orthostatic hypotension through alpha-adrenergic blockade, and this adds to the blood pressure lowering of amlodipine and benazepril. Dizziness, syncope and falls are more likely, particularly at the start of treatment or after dose increases.
What this interaction means in plain words
Amlodipine-benazepril and Asenapine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-benazepril belongs to Cardiovascular; Asenapine belongs to Central Nervous System.
Why it happens
Additive pharmacodynamic hypotensive effects from alpha-1 antagonism by asenapine and vasodilation/RAAS inhibition by the antihypertensives.
What to do
Monitor blood pressure and orthostatic symptoms when asenapine is started or titrated. Consider reducing the antihypertensive dose if symptomatic hypotension occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
