Summary
Isoflurane causes dose-dependent vasodilation and myocardial depression, and in patients taking amlodipine and benazepril this can produce marked intraoperative hypotension. ACE inhibitors in particular are associated with refractory hypotension under general anesthesia.
What this interaction means in plain words
Amlodipine-benazepril and Isoflurane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-benazepril belongs to Cardiovascular; Isoflurane belongs to Central Nervous System.
Why it happens
Additive vasodilation and reduced vascular tone from volatile anesthetic, calcium channel blockade and suppressed angiotensin II.
What to do
Inform the anesthesia team of the antihypertensives and consider holding the ACE inhibitor on the morning of surgery per local protocol. Monitor blood pressure closely and treat hypotension with fluids and vasopressors.
