Summary
ACE inhibitors reduce aldosterone and spironolactone blocks its receptor, so the two together can cause marked potassium retention. Severe hyperkalemia and arrhythmias can occur, especially with renal impairment or diabetes.
What this interaction means in plain words
Amlodipine-benazepril and Spironolactone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amlodipine-benazepril belongs to Cardiovascular; Spironolactone belongs to Cardiovascular.
Why it happens
Additive reduction in renal potassium excretion via suppressed aldosterone production and mineralocorticoid receptor blockade.
What to do
Check serum potassium and creatinine at baseline, within the first week or so, and periodically thereafter. Avoid potassium supplements and reduce or stop spironolactone if potassium rises significantly.
