Summary

Tolmetin, an NSAID, can blunt the blood pressure-lowering effect of benazepril. Together they may also reduce renal function, particularly in older, volume-depleted or already renally impaired patients, and can cause acute kidney injury.

What this interaction means in plain words

Amlodipine-benazepril and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-benazepril belongs to Cardiovascular; Tolmetin belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAIDs inhibit synthesis of vasodilatory renal prostaglandins, which opposes ACE inhibitor effects and lowers glomerular filtration pressure.

What to do

Prefer a non-NSAID analgesic such as paracetamol where possible. If an NSAID is needed, keep hydration adequate and monitor blood pressure, serum creatinine and potassium after starting and during therapy.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
