Summary

Candesartan is a second angiotensin receptor blocker given on top of valsartan, which provides dual renin-angiotensin system blockade without added benefit. This raises the risk of hypotension, hyperkalemia and renal impairment, although the thiazide component partly offsets potassium retention.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Candesartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Candesartan belongs to Cardiovascular.

Evidence level: High

Why it happens

Duplicate angiotensin II type 1 receptor blockade produces additive hypotensive, potassium-retaining and renal hemodynamic effects.

What to do

Avoid combining two ARBs and use a single agent at an appropriate dose. If it has happened, check potassium and creatinine promptly and stop one of the drugs.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
