Summary
Carbenoxolone has mineralocorticoid-like activity causing sodium retention and potassium loss. With hydrochlorothiazide, hypokalemia is more likely and sodium and fluid retention may reduce blood pressure control. Valsartan only partly offsets potassium loss.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Carbenoxolone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive renal potassium wasting from thiazide and carbenoxolone's mineralocorticoid effect, which also opposes antihypertensive therapy.
What to do
Monitor serum potassium and blood pressure, and replace potassium as needed. Avoid the combination in patients with heart disease or on digoxin.
