Summary

Cortisone has mineralocorticoid activity that promotes potassium loss, which adds to the potassium-wasting effect of hydrochlorothiazide. This raises the risk of hypokalemia and, in some patients, fluid retention that opposes the antihypertensive effect.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Cortisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive renal potassium wasting and sodium retention from corticosteroid and thiazide actions.

What to do

Monitor serum potassium and blood pressure, and replace potassium as needed. Valsartan may partly offset potassium loss.
