Summary
NSAIDs such as fenbufen can reduce the antihypertensive effect of valsartan and hydrochlorothiazide and impair renal function, particularly in older or volume-depleted patients. Hyperkalemia and acute kidney injury may occur with an ARB. The thiazide component adds to the volume-depletion risk.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Fenbufen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
NSAIDs inhibit renal prostaglandin synthesis, reducing renal perfusion, promoting sodium retention and impairing potassium excretion alongside angiotensin receptor blockade.
What to do
Monitor blood pressure, serum creatinine and potassium, and keep the patient hydrated. Use the lowest NSAID dose for the shortest time or choose an alternative analgesic.
What to ask your pharmacist
