Summary
NSAIDs such as meclofenamate can blunt the antihypertensive effect of valsartan and hydrochlorothiazide. In volume-depleted or older patients, the combination of an NSAID, an ARB and a diuretic increases the risk of acute kidney injury and hyperkalemia.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Meclofenamate can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
NSAIDs inhibit renal prostaglandin synthesis, causing sodium and water retention and reduced renal perfusion, which opposes diuretic and ARB effects and impairs glomerular filtration.
What to do
Avoid or limit NSAID use where possible, ensuring adequate hydration. Monitor blood pressure, serum creatinine and potassium, particularly early in therapy.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
