Summary

This is a duplication of thiazide therapy, since the combination product already contains hydrochlorothiazide. It increases the risk of volume depletion, hypotension, hyponatremia, hypokalemia and renal impairment, partly offset by valsartan's potassium-sparing effect.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Methyclothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Two thiazide diuretics produce additive natriuresis and potassium wasting.

What to do

Avoid combining two thiazides and stop the redundant agent. If unavoidable, monitor electrolytes, renal function and blood pressure.
