Summary
Both hydrochlorothiazide and beta-2 agonists can lower serum potassium. Together they may produce clinically relevant hypokalemia, which can promote arrhythmias, particularly in patients with cardiac disease or hypoxia. Systemic exposure to inhaled olodaterol is low, so the effect is usually modest.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Olodaterol inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive potassium lowering: thiazide-induced renal potassium loss plus beta-2 agonist-driven intracellular potassium shift.
What to do
Check serum potassium at baseline and periodically, especially in patients with severe COPD or cardiac disease. Replace potassium if levels are low.
