Summary

Hydrochlorothiazide may reduce renal tubular secretion of procainamide and its metabolite NAPA, raising their levels. Thiazide-induced hypokalemia may also increase the risk of proarrhythmia.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Procainamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Competition for renal tubular cationic secretion, plus thiazide-induced electrolyte disturbance.

What to do

Monitor ECG, serum procainamide and NAPA levels and serum potassium. Reduce the procainamide dose if levels or QRS or QT intervals increase.
