Summary

This combination duplicates valsartan, since the product already contains it and sacubitril/valsartan adds a second ARB dose. This increases the risk of hypotension, hyperkalemia, renal impairment and angioedema. Dual ARB exposure offers no added benefit.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Sacubitril / valsartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Duplicate angiotensin II receptor blockade with additive hypotensive and potassium-retaining effects.

What to do

Avoid concurrent use and replace the fixed-dose combination with agents lacking an ARB (such as amlodipine with hydrochlorothiazide) if sacubitril/valsartan is needed. If overlap is unavoidable, monitor blood pressure, potassium and creatinine closely.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
