Summary
Non-acetylated salicylates can reduce renal prostaglandin synthesis, which may blunt the antihypertensive effect of valsartan and hydrochlorothiazide and impair renal function in volume-depleted or elderly patients. Potassium effects are unpredictable because the ARB tends to raise potassium while the thiazide lowers it.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Salicylates can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of vasodilatory renal prostaglandins by salicylates opposes ARB and diuretic effects and reduces renal perfusion.
What to do
Monitor blood pressure, serum creatinine and potassium when starting or changing salicylate therapy, and ensure adequate hydration. Consider a non-NSAID analgesic if blood pressure control or renal function worsens.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
