Summary
Salsalate, a salicylate NSAID, can diminish the blood pressure lowering effect of valsartan and hydrochlorothiazide. Combined with an ARB and a diuretic it raises the risk of acute kidney injury, especially in elderly or volume-depleted patients, and may contribute to hyperkalemia.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Salsalate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Reduced synthesis of vasodilatory renal prostaglandins lowers glomerular perfusion and counteracts antihypertensive and natriuretic effects.
What to do
Monitor blood pressure, renal function and potassium within the first weeks of combined use and maintain hydration. Use the lowest effective salsalate dose for the shortest time, or choose an alternative analgesic.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
