Summary

Diflunisal can blunt the blood pressure lowering effect of olmesartan. In combination with an angiotensin receptor blocker, NSAIDs can also reduce glomerular filtration and cause acute kidney injury, especially in elderly or volume-depleted patients, and may raise serum potassium.

What this interaction means in plain words

Amlodipine-olmesartan and Diflunisal can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-olmesartan belongs to Cardiovascular; Diflunisal belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, opposing ARB effects and impairing glomerular filtration.

What to do

Monitor blood pressure, serum creatinine and potassium when starting or stopping diflunisal, and ensure adequate hydration. Use the lowest effective NSAID dose for the shortest time, or choose an alternative analgesic such as paracetamol.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
