Summary

NSAIDs can blunt the antihypertensive effect of olmesartan and, together with an angiotensin receptor blocker, raise the risk of acute kidney injury and hyperkalemia. The risk is greatest in elderly, volume-depleted or renally impaired patients.

What this interaction means in plain words

Amlodipine-olmesartan and Oxaprozin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-olmesartan belongs to Cardiovascular; Oxaprozin belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAIDs inhibit vasodilatory renal prostaglandins, causing sodium retention and reduced glomerular perfusion while the ARB lowers efferent arteriolar tone.

What to do

Use the lowest effective NSAID dose for the shortest time and ensure adequate hydration. Monitor blood pressure, serum creatinine and potassium, especially at the start.

What to ask your pharmacist
