Summary
Beta-blockers can oppose the bronchodilator action of long-acting beta-2 agonists such as arformoterol, and may precipitate bronchospasm in patients with airway disease. Nebivolol's beta-1 selectivity lowers but does not eliminate this risk.
What this interaction means in plain words
Arformoterol and Nebivolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Arformoterol belongs to Respiratory; Nebivolol belongs to Cardiovascular.
Why it happens
Competitive pharmacodynamic antagonism at beta adrenergic receptors, with loss of selectivity at higher nebivolol doses.
What to do
Monitor respiratory status and heart rate, keep the nebivolol dose low, and consider an alternative to beta-blockade in patients with significant bronchospasm.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
