Summary
Aspirin irreversibly inhibits platelets and can cause gastrointestinal bleeding. Together with argatroban, bleeding risk is clearly increased even with the rectal route, as systemic absorption occurs.
What this interaction means in plain words
Argatroban and Aspirin rectal can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive hemostatic impairment: irreversible COX-1 platelet inhibition plus direct thrombin inhibition.
What to do
Combine only when there is a clear indication such as a recent stent, and monitor for bleeding and hemoglobin. Otherwise avoid antiplatelet doses of aspirin during argatroban.
What to ask your pharmacist
