Summary

Aripiprazole has alpha-1 adrenergic blocking activity, which may blunt the pressor response to epinephrine and cause unopposed beta-mediated vasodilation and hypotension. This is mainly relevant when epinephrine is given systemically, such as in anaphylaxis or resuscitation.

What this interaction means in plain words

Aripiprazole and Epinephrine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Aripiprazole belongs to Central Nervous System; Epinephrine belongs to Endocrine.

Evidence level: Low

Why it happens

Alpha-1 adrenergic antagonism by aripiprazole counteracts the vasoconstrictor effect of epinephrine.

What to do

Monitor blood pressure closely if systemic epinephrine is given and use a pure alpha-agonist vasopressor such as norepinephrine or phenylephrine if hypotension persists.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
