Summary
Sparsentan is a dual endothelin and angiotensin II receptor antagonist, so adding an ARB duplicates angiotensin receptor blockade. This increases the risk of hypotension, syncope, hyperkalemia and acute kidney injury.
What this interaction means in plain words
Azilsartan-chlorthalidone and Sparsentan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Azilsartan-chlorthalidone belongs to Cardiovascular; Sparsentan belongs to Genitourinary.
Why it happens
Additive angiotensin II type 1 receptor blockade with duplicated pharmacodynamic effects.
What to do
Do not coadminister; discontinue the ARB before starting sparsentan and monitor blood pressure, potassium and renal function.
