Summary

Combining baclofen with a tricyclic antidepressant can increase CNS depression and anticholinergic burden. Muscle hypotonia, confusion, and urinary retention may occur, and in rare cases baclofen with tricyclics has been associated with marked muscle weakness. Protriptyline is less sedating than other tricyclics, but additive effects can still occur.

What this interaction means in plain words

Baclofen and Protriptyline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive CNS depressant and pharmacodynamic effects of baclofen and the tricyclic antidepressant.

What to do

Monitor for sedation, weakness, confusion and urinary retention, particularly after dose changes. Reduce the baclofen dose if excessive hypotonia or sedation develops.
