Summary
Rifampin induces metabolic enzymes and can lower exposure to both bazedoxifene and the estrogens. Reduced bazedoxifene exposure may weaken endometrial protection, raising the risk of endometrial hyperplasia, and estrogen efficacy may also fall.
What this interaction means in plain words
Bazedoxifene-conjugated estrogens and Rifampin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Induction of CYP3A4 and UGT enzymes by rifampin increases clearance of bazedoxifene and estrogens.
What to do
Avoid the combination and use an alternative therapy if rifampin is required. If it cannot be avoided, monitor for breakthrough bleeding and loss of symptom control.
